TL;DR

Medicare pays for skilled nursing after a qualifying three-day inpatient stay; observation days do not count. You may choose any Medicare-certified home with a bed. Compare CMS staffing, turnover and the latest inspection, and weigh distance from family.

Why discharge decisions are rushed

Hospitals are paid a set amount for most Medicare admissions, so once a patient is medically stable, the case management team moves quickly. Families are often handed a list of nursing homes that have accepted the referral and asked to choose by the afternoon.

You have more say than it feels like. Medicare patients may choose any Medicare-certified skilled nursing facility that has a bed and accepts them, and the hospital must give you a list. You can ask for time to look at the options, and you can ask whether your parent was admitted as an inpatient or kept under observation, which decides whether Part A will pay for the nursing home.

Which nursing homes are near each hospital?

Distance matters for family visits and for follow-up appointments with the hospital's surgeons. Here are the three closest licensed nursing homes to each acute-care hospital in the two counties, by straight-line distance, with CMS's overall rating:

What should you compare in an hour?

Open each candidate's page and look at three numbers: total nurse staffing hours per resident per day against the two-county median of 3.69; staff turnover against the median of 45.6%; and the number of deficiencies at the latest health inspection against the median of 6. Then check CMS's short-stay quality rating, which reflects rehab patients specifically.

If your parent has Medicare Advantage, call the plan: it may limit you to network homes and require prior authorization.

What if assisted living or home is the better next step?

Some patients do not need a nursing home at all. A patient who can transfer with help and does not need 24-hour nursing can go to an assisted living facility, with home health visits ordered to the ALF address. A patient going home can receive Medicare home health if homebound and in need of skilled care.

Ask the hospital team which of those is realistic before agreeing to a nursing home placement, and line up help for the first nights at home, which are when most readmissions start.

What if the nursing home you want has no bed?

Ask to be placed on its waiting list and choose the best available alternative for now; once your parent is in a skilled nursing facility, a transfer to another home is possible when a bed opens, though it means another move during recovery. For short rehab stays, many families decide the second move is not worth it.

If a bed will open in a day or two, ask the hospital whether your parent can stay; the answer depends on medical need and the plan.

How do you know when rehab will end?

Medicare coverage continues only while your parent needs and receives daily skilled care. The nursing home must give written notice, usually two days ahead, when it believes Medicare coverage will end, and you have the right to a fast appeal through Medicare's quality improvement organization.

Use the rehab stay to plan the next step: return home with help, assisted living, or long-term nursing care paid privately or by Medicaid.

Frequently asked questions

Can I choose which nursing home my parent goes to after the hospital?

Yes; Medicare patients may choose any certified skilled nursing facility with a bed that accepts them.

Does observation status affect Medicare nursing home coverage?

Yes; only inpatient days count toward the three-day requirement for Part A skilled nursing coverage.

How long does Medicare pay for rehab?

Up to 100 days per benefit period while daily skilled care is needed, with coinsurance after day 20.

Do Medicare Advantage plans pay for rehab?

Yes, under their own rules; many waive the three-day stay but require prior authorization and network facilities.
